To receive free (OHIP covered) consultation, you required to fill out the registration patient intake form for all your inquiries, and then the first visit patient intake form if you are a first-time patient. You should fill out follow up patient intake form if you are revisiting us. All of the visits required referral from your doctor/midwife or nurse practitioner.
